My students get roughly three bathroom breaks each day (whether they choose to use this or not is their choice). They can also go during recess or lunch if needed. Finally, I have a sign out system for students to use the bathroom once any other time during the day. So basically, they get a morning, early afternoon, and late afternoon break - plus one of their own choice. And it's pretty easy to tell when there's an "emergency" and students might need an additional time. Fortunately, my students don't abuse this, so it hasn't been a problem.
